General Informaion

Pomona Valley Hospital Medical Center’s Diabetes Self-Management Training offers a comprehensive education program designed to equip newly diagnosed diabetes patients and those with uncontrolled diabetes with the tools to achieve and maintain good health. Recognized by the American Diabetes Association, the staff consists of Certified Diabetes Educators, Registered Nurses and Registered Dietitians.

Cardiac Wellness Offices (Basement of hospital)
When:
Tuesday and Thursday afternoons, 1:00 p.m. – 2:30 p.m.
Wednesday evenings, 5:00 p.m. – 6:30 p.m.

A Series of Five 90 Minute Classes includes
  • Diabetes disease process
  • Nutritional management of blood glucose
  • Medications
  • Preventing, detecting, and treating acute complications
  • Monitoring blood glucose
  • Preventing, detecting, and treating chronic complications
  • Heart healthy eating
  • Physical activity
  • Goal setting and problem solving for daily living
  • Psychological adjustment

Physician Referral Required…Cost covered by many insurance plans.
Call the Stead Health and Fitness Center to schedule an appointment.

On-going education is available through the monthly Diabetes Support Group, which meets the third Thursday of each month at 5:00 p.m. A light meal is served for $5.00 per person. Call (909) 630-7333 for reservations.
