The Pomona Clinic Coalition
The County of Los Angeles and
local area healthcare providers have joined together to form the Public/Private
Partnership - Pomona Clinic Coalition (PCC) offering primary
health care services.
Through the Pomona Clinic Coalition
residents within the local community are able to access local preventative
and primary health care services. The PCC was formed to improve and
protect
the health of local families through reliable, consistent
and accessible medical care.
Who can use this service and how
much does it cost?
If you live in the local community, do not have medical insurance, and/or
can not afford medical care, you can qualify for the service. The cost
is based on your income. No one will be refused treatment because of lack
of ability to pay. A financial screening will qualify you for: The Public/Private
Partnership (no cost); a Sliding Fee Schedule; the Medi-Cal Program; or
the Medi-Cal Healthy Families Program.
How to Enroll
Pomona Valley Hospital Medical Center (PVHMC) is the coordinating Lead
Agency for the Pomona Clinic Coalition and a Nurse Practitioner and
family Practioner Doctor provide medical care at the Comminity Health
Center. PVHMC staff will assist with enrollment. Participating
clinics offer
English
-Spanish
