The Transitional Care Unit (TCU) at Pomona Valley Hospital
Medical Center provides short-term health care for patients who have
continued medical care needs greater than home care but less than acute
care hospitalization. Customized therapeutic programs are developed for
patients in the TCU to speed and maximize rehabilitation. The TCU is
licensed as a skilled nursing facility housed within the hospital.
Specialized services within the TCU are offered in a more homelike
environment. These services are tailored to promote quality living and
to encourage participation in recovery.
Components of the program include:
Nursing Care
Under the direction of your personal physician, extensive nursing interventions
are provided. Highly skilled nursing care is provided 24 hours a day to
meet the individualized needs of each patient.
Physical Therapy
The TCU works in conjunction with the hospital’s Physical Therapy
and Rehabilitation Department to provide comprehensive rehabilitation
services. A complete satellite gym within the TCU provides specialized
equipment to allow for comprehensive exercise programs. Progressive wound
care including hydrotherapy is also available. These services improve
functional independence and insure a smooth transition upon discharge
to home or a specific facility.
Occupational Therapy
The Occupational Therapy program encourages independence in daily living
activities such as self care and home management. Treatments may focus
on adaptive equipment training, energy conservation/work simplification
techniques and cognitive and perceptual motor skills.
Speech Therapy
Individually designed speech therapy programs focus on communication
skills including: comprehension, speech, expressive language, voice,
reading and writing. Services also include treatment for patients with
swallowing impairments.
Activity Program
The Activity Program encourages the participation of each patient to
meet individual needs and interest to support the healing process on
all levels: social, physical and emotional. The program includes: social,
physical and mental activities; creative classes such as arts, crafts
and cooking; special holiday events; religious and musical programs.
The TCU focuses on each patient’s unique needs and goals for recovery.
