PVHMC and Anthem Blue Cross have reached an agreement that allows HMO, PPO and EPO members to once again receive in-network care at our Hospital. You can expect a seamless transition back to PVHMC, in network co-pays and quality healthcare when you need it, close to home. What does it mean now that PVHMC has a contract with Anthem Blue Cross?
The reinstatement means that patients with Anthem Blue Cross health insurance plans will have access to in-network care at PVHMC. The contract covers both HMO and PPO Anthem Blue Cross plans. Members enrolled in these plans can receive care at PVHMC for covered medical services at the highest level of benefits.

How long is the new contract with Anthem Blue Cross?
Pomona Valley Hospital Medical Center (PVHMC) and Anthem Blue Cross have reached a multi-year agreement. Physicians and specialists can be confident in referring patients with Anthem Blue Cross insurance to PVHMC.

Does PVHMC accept any and all insurance plans?
PVHMC will provide emergency care and stabilization for all patients regardless of their ability to pay.

PVHMC accepts self-pay, Medicare, Medicare/Medi-Cal (Medi-Medi) fee-for-service and most managed care insurance plans, including most HMO, PPO and POS plans, even many health plans we don't contract with. By accepting nearly all insurance options, our entire team of medical professionals works together with your health plan to ensure the highest quality care in the most efficient, cost effective manner.

Although your health insurance plan may be listed, your coverage is determined by your health insurance plan.

What services are covered by the new contract? What services are not covered?
PVHMC is now contracted with Anthem Blue Cross as an in-network provider. In some cases, you may need authorization for scheduled services. Please discuss authorization with your physician’s office. He or she may need to contact Anthem Blue Cross and/or the IPA.

If emergency services are needed, can PVHMC provide those?
Regardless if you have insurance or not, or the ability to pay, if you need medical treatment you should go to the nearest medical emergency facility.
