Appointments are usually made within 48 hours of the athlete’s call. Athletes under the age of 18 must be accompanied by an adult or have a signed consent form. SMC Physician Clinic injury screens include an x-ray (if needed) and are free of charge, and therefore do not require insurance information. Pre-participation examinations performed at the SMC cost $31.00.
Rehabilitation treatments, Urgent Care services, and Radiology will require either insurance or cash payment. Most insurance plans are accepted, including Blue Cross PPO, but occasionally insurance authorization must be obtained prior to the first visit.
