Hospital Admitting / Pre-registration

At the beginning of your third trimester, we encourage you to pre-admit for your hospital stay. This ensures your information is in our system before your special day arrives.

You may accomplish this at the hospital.

You will need to bring the following:

Identification:

Insurance/medical card and social security card Work address and phone number Name, address, and telephone number of an emergency contact Completed pre-admission form Physician & pediatrician information

Registration office is available in the following areas:

• Main lobby of the hospital M-F 5am to 8pm
• Radiology department M-F 6:30am to 6pm and Sat 7am to 2pm
• Emergency Room Open 24 hours a day, 7 days a week

In addition, registration is available at the Maternity Orientation (one hour prior to tour).

During the pre-admission process at the hospital, we provide you with more details and answer any questions you have about your hospital stay.

No appointment is necessary.
