REHABILITATION
A stroke or Cerebral Vascular Accident (CVA) can be a
life-changing event. Proper care, from the outset, is critical in maximizing
recovery of function in these patients. All patients with neurological
conditions are treated by Neuro Developmental Techniques (NDT) trained
and certified therapists who focus on measurable, functional improvements
in those areas most necessary for regaining independence. Stroke Rehabilitation
at PVHMC offers a continuum of care available to patients as they progress
from inpatient status ... to the Transitional
Care Unit (TCU) ... and
through their outpatient recovery.
Inpatient Rehabilitation…
If
a stroke requires admission to the hospital, rehabilitation begins immediately.
Highly experienced therapists specially trained in stroke rehabilitation
develop and share with the patient the plan of treatment and expectations.
Our long-term goal is to return patients to their maximum level of function,
emphasizing safety, education and independence. The hospital stay is
usually brief and when appropriate, we will facilitate a smooth transfer
to the Transitional Care Unit (TCU) for more comprehensive and aggressive
rehabilitation.
The Transitional Care Unit…
The Transitional Care
Unit (TCU) on the sixth floor offers many benefits. Physicians can conveniently
follow their patient’s progress. Communication and continuity between
patients and therapists while discussing progress and planning upcoming
treatment goals is enhanced. Families are encouraged to attend both the
treatment and the interdisciplinary meetings to discuss the patient’s
progress and ask questions. Therapists on the TCU have advanced training
and experience in stroke rehabilitation. Custom treatments using specialized
equipment and techniques are tailored to each patient and the focus is
on returning patients to their prior level of function. The patient,
family, and “rehabilitation team” (Physical Therapist, Occupational
Therapist, Speech-Language Pathologist, Nurse, Discharge Planner, and
Physician) all work together to achieve the most successful outcome possible.
Our focus is on achieving meaningful, measurable, and functional improvements
while minimizing the length of stay. Prior to discharge, family or caretaker
training will be arranged to ensure a safe transition to home.
Outpatient
Rehabilitation…
Upon discharge from the TCU, the rehabilitation
team determines the patient’s need for ongoing rehabilitation as
an outpatient at Pomona Valley Hospital Medical Center’s Department
of Physical Therapy and Rehabilitation. Our comprehensive outpatient
rehabilitation services include Physical, Occupational and Speech Therapy … all
offered with the highest quality of care and the convenience of timely
communication between therapists who treat patients during their inpatient
or TCU stays and those who provide outpatient treatment. Patient and
family goals are established and progress is assessed during each treatment.
With advanced handling techniques and high-tech equipment such as the
Balance Master by Neurocom, therapists can challenge the patient’s
potential for maximal recovery.
