| Thank you for visiting the Diabetes Self-Management Training Program at Pomona Valley Hospital Medical Center.
Diabetes is a life-long disease affecting 21 million Americans. Diabetes impairs your body's ability to properly utilize the energy from the food you eat. It can result in uncontrolled blood sugars and abnormal lipids leading to both short and long-term complications. Pre-diabetes exists when your blood sugar is higher than normal but not yet in the diabetes range. Our diabetes program is nationally recognized by the American Diabetes Association. We have a multidisciplinary staff of diabetes educators, including a Spanish-speaking educator. We serve adults with pre-diabetes, as well as type 1 and type 2 diabetes. Patients are taught self-management skills and are encouraged to implement self-care behaviors to minimize the risk of complications. Initially, we provide five hours of training:
