| Heart failure (cardiomyopathy) remains a major health issue in the United States. It is estimated that five million Americans have symptomatic heart failure and remains the number one reason for being admitted to the hospital in the United States. A half million new cases are diagnosed every year, and as the population ages in the United States, the incidence and prevalence of heart failure will continue to increase.
Care of patients with heart failure continues to evolve with newer therapeutic options including newer drugs and devices. The Stead Heart and Vascular Center has a comprehensive heart failure program overseen by an extensively trained heart failure case manager, and nationally recognized by the American Heart Association’s Get with the Guidelines Program. The case manager is supported by nurses and nurse practitioners for a team approach in the care of these patients. Presently, we offer M.D. follow-up, nurse practitioner follow-up, and heart failure education to help the patient understand their disease process. The patient education module includes diet, exercise, and medication compliance. Patients have direct access to HF nurses and nurse practitioners to discuss their problems and concerns.
