In partnership with your physician, we trust this program will give you
support and information to help you with your heart failure recovery.
Since heart failure, like diabetes or arthritis, is a chronic disease,
our goal is to manage the disease and its symptoms. Although some days
are more difficult than others, remaining successful in controlling the
disease means not being overwhelmed or discouraged by day-to-day changes.
The best way is to notice these changes in signs and symptoms and to address them.
Many people with heart failure lead normal, active lives. They do so because
they have learned to take good care of themselves. Our Heart Failure Patient
Education Program is designed to help you learn to live successfully with
heart failure. We want you to learn about your body, the disease, and
methods of treatment. You can take control of heart failure by carefully
following proper medication advice and healthy lifestyle changes. If you
do, you may be able to lead an active and satisfying life.
Our program provides detailed information, group lectures and peer interaction
for individuals with heart failure and will help you to:
- Understand your treatment plan
- Learn how to actively work with your doctor or nurse so you can feel better
- Learn how to overcome common stumbling blocks in following the treatment
plan of your life
We have a wide variety of resources and information all designed for you.
Our Heart Failure Education and Awareness group learn about the many simple
aspects of heart failure that are easily controlled. Working together
we can better meet your needs and manage your heart failure. Our goal
is for you to be comfortable in your daily activities and enjoy life.
