Heart Failure Program

Welcome to the Stead Heart & Vascular Center’s Heart Failure Program at Pomona Valley Hospital Medical Center. In partnership with your physician, we trust this program will give you support and information to help you with your heart failure recovery.

Since heart failure, like diabetes or arthritis, is a chronic disease, our goal is to manage the disease and its symptoms. Although some days are more difficult than others, remaining successful in controlling the disease means not being overwhelmed or discouraged by day-to-day changes. The best way is to notice these changes in signs and symptoms and to address them.

We have a wide variety of resources and information all designed for you. Our Heart Failure Education and Awareness group learn about the many simple aspects of heart failure that are easily controlled. Working together we can better meet your needs and manage your heart failure. Our goal is for you to be comfortable in your daily activities and enjoy life.
