The Stead Heart & Vascular Center's Heart Failure Program at Pomona
Valley Hospital Medical Center (PVHMC) is designed to help patients deal
with and recover from heart failure. We partner with your primary physician
in order to provide the best possible care, treatment, and education for
you. Our program wants to equip you to live a healthy, active life as
you effectively manage your heart disease.
Heart failure is not a prison sentence-many people with heart failure lead
happy, fulfilled lives because they learn how to pay attention to their
bodies and how to take care of themselves. Through healthy lifestyle changes
and proper medication, you can overcome your heart failure and learn how
to lead an active life in spite of it.
Our program specializes in education and peer support for those living
with heart failure. We can help patients:
- Understand their treatment plan
- Work with their doctor to feel better daily
- Overcome common obstacles to follow the treatment plan
Our support groups help provide mutual support and advice to manage the
simple aspects of heart failure that are able to be controlled. By understanding
your disease and your health, we can help you begin building the routine
that results in the highest quality of life for you.
