PVHMC Receives National Recognition for Providing High Quality Heart Failure and Stroke Care

Pomona Valley Hospital Medical Center (PVHMC) has received two gold level American Heart Association / American Stroke Association Get With The Guidelines® achievement awards for demonstrating commitment to following up-to-date, research-based guidelines for the treatment of heart disease, diabetes, and stroke, ultimately leading to more lives saved, shorter recovery times and fewer readmissions to the hospital.
This year, PVHMC received:
- Heart Failure Gold Plus Award
- Target: Heart Failure Honor Roll, 15th year in a row
- Target: Type 2 Diabetes Honor Roll, 4th year in a row
- Stroke Gold Plus Award
- Target: Stroke Honor Roll Elite Plus, 9th year in a row
- Target: Type 2 Diabetes Honor Roll, 2nd year in a row
Heart disease and stroke are the No. 1 and No. 5 causes of death in the United States, respectively, according to the American Heart Association 2025 Statistical Update. Studies show patients can recover better when providers consistently follow treatment guidelines.
Get With The Guidelines® put the expertise of the American Heart Association and American Stroke Association to work for hospitals nationwide, helping ensure patient care is aligned with the latest evidence- and research-based guidelines for cardiovascular conditions. The honor roll recognition ensures patients with type two diabetes receive the most up-to-date, evidence-based care when hospitalized with these conditions. As a participant in Get With The Guidelines® programs, PVHMC qualified for the award by demonstrating how their organization has committed to improving quality care.
