Welcome to PVHMC’s MYHEALTH Patient Portal, a safe, secure and innovative health engagement tool that empowers patients to take a more active role in managing their healthcare needs.

Our MYHEALTH patient portal allows you to securely view, download and transmit your health information, 24/7. This no-cost online tool is available to all patients at Pomona Valley Hospital Medical Center and our Pomona Valley Health Centers in Claremont, Chino Hills, La Verne, Pomona and Upland. And, with our HealtheLife app, you can view your health information anywhere, anytime.
