PVHMC Earns Sixth ‘A’ Hospital Safety Grade from The Leapfrog Group

Pomona Valley Hospital Medical Center (PVHMC) has earned its sixth consecutive “A” Hospital Safety Grade from The Leapfrog Group, an independent national nonprofit watchdog focused on patient safety, six consecutive reporting periods.
Leapfrog assigns an “A,” “B,” “C,” “D” or “F” grade to general hospitals across the country based on over 30 measures of errors, accidents, injuries and infections as well as the systems hospitals have in place to prevent them.
“This sixth consecutive “A” grade distinction reflects our dedication to the highest standards in patient safety,” said Richard E. Yochum, FACHE, Pomona Valley Hospital Medical Center President/CEO. “It is a testament to the collective leadership of our Associates and physicians in delivering care our community trusts and relies on.”
The hospital implements countless programs and initiatives spanning all departments and units, including medication safety, hand hygiene, feedback and intervention processes, infection prevention and many more, to ensure patients receive the best possible care and outcomes.
The Leapfrog Hospital Safety Grade stands as the only hospital ratings program focused solely on preventable medical errors, infections and injuries that kill more than 500 patients a day in the United States. This program is peer-reviewed, fully transparent and free to the public. Grades are updated twice annually, in the fall and spring.
