Your Voice Matters! Participate in Pomona Valley Hospital Medical Center’s 2024 Community Needs Assessment
Have you ever wanted to share your thoughts and impact the health services and programs in your community? Now is your chance.
Every three years, Pomona Valley Hospital Medical Center (PVHMC) partners with California State University, San Bernardino’s Institute of Applied Research to conduct a formal Community Needs Assessment (CNA) to identify the status of health and wellness of our community.
The assessment is intended to be a resource to examine and prioritize critical health needs, as well as our current programs and strategies, and to trigger the development of activities and programs that can help enhance the health and well-being of the residents throughout the Pomona Valley.
Improvements from the 2021 CNA resulted in improvements to the following critical needs:
- Diabetes Prevention and Management
- Substance Abuse Interventions
- Chronic Illness and Disease Prevention
For more than 120-years, we have dedicated a profound commitment to our community and welcome this occasion to formalize the Community Benefit Plan and Implementation Strategy. Our community is central to our mission of providing high quality, cost effective health care services and we strive continuously to improve the status of health by reaching out and serving the needs of our diverse ethnic, religious and cultural community.
