Pomona Valley Hospital Medical Center and our affiliated Clinics strive to fulfill your request promptly, ensuring your privacy. • Access your medical record via MYHEALTH Patient Portal
• Request your medical records electronically. Click here.
  • Documents needed:
    • Your photo identification
    • Legal documents (if applicable)
• Complete the medical record request authorization form and submit via email, fax, or mail
  • Download and print the authorization form for the Release of Health Information Form
  • Complete and sign the form (electronic signatures are not accepted currently)
  • Provide a copy of your photo identification
  • Legal document (if applicable)

Patients may obtain medical records on a walk-in basis at Pomona Valley Hospital Medical Center.
