Obtaining Your Medical Records

Your medical records help you and your physician keep careful track of your health and well-being. We are here to provide you with the information you need. Pomona Valley Hospital Medical Center and our affiliated Clinics strive to fulfill your request promptly, ensuring your privacy. The following information will help guide you through the available options, provide FAQs, and offer contact information if you have any questions.

  • Access your medical record via MYHEALTH Patient Portal
  • Request your medical records electronically. Click here.
    • Documents needed:
      • Your photo identification
      • Legal documents (if applicable)
  • Complete the medical record request authorization form and submit via email, fax, or mail
    • Download and print the authorization form for the Release of Health Information Form
    • Complete and sign the form (electronic signatures are not accepted currently)
    • Provide a copy of your photo identification
    • Legal document (if applicable)

Patients may obtain medical records on a walk-in basis at Pomona Valley Hospital Medical Center.

Business Hours: Monday – Friday 8:00 am – 4:30 pm
