Health and Wellness App

Please complete the form below to request access to patient health information using the app of your choice that is not currently supported by PVHMC. Below are the apps that can be used to access your PVHMC health records via API (Application Programming Interface). Note that this form is not to be used for requesting medical records.

PVHMC has API connections with the following health apps to view your records with ease:

We are connected to Apple Health. Download the Patient Guide for Apple Health Data Share with Provider IOS 15 and Higher.

Data available in Apple Health includes: Demographics, allergies, immunizations, lab results, medications, problems, procedures and vital signs.

To connect to Apple Health:

  1. Go to the Apple health app and click “Health Records”
  2. In the search box type “Pomona Valley Hospital Medical Center”
  3. In the results select “Pomona Valley Hospital Medical Center”
  4. On the next screen, you will see a section titled “Available to Connect”. Select “Pomona Valley Hospital Medical Center”
  5. You will be required to input your MYHEATH user name and password
  6. Review access terms before allowing access, then select “Allow Access” at the bottom of the screen
  7. You will see a screen that says “Account Added”. Click “Done”
  8. Your MYHEATH data should now be available under Health Records in your Apple Health App.
