Our convenient outpatient clinics serve the communities surrounding Chino Hills, Claremont, La Verne and Pomona. Each clinic offers individualized treatment plans, computerized testing and exercise equipment, modalities such as whirlpool, neuromuscular electrical stimulation, ultrasound, traction, patient education - and because each is a PVHMC Sports Medicine Center rehab location, our highly trained and experienced therapists know what athletes need to recover from injury and return to play quickly and safely.

Proudly renamed as the Charles M. Magistro Physical Therapy and Rehabilitation Center, our main department is on the first floor of the Robert and Beverly Lewis Outpatient Pavilion located adjacent to PVHMC. Designed with both rehab and wellness in mind, our beautiful new department offers a spacious state-of-the-art rehab gym along with private treatment rooms and designated areas for occupational therapy, speech therapy, cardio-pulmonary rehab, neurologic rehab, and wound care- all with floor to ceiling windows to provide lots of natural lighting. Ample parking with easy access is available in front of the pavilion. This location is also home to our Wound Care Center and Sports Medicine Center.
Our Claremont Rehabilitation Clinic has been serving the community since 1983. Located on the second floor of the Pomona Valley Health Center at the corner of Monte Vista and Claremont Blvd, this spacious clinic provides a panoramic view of the scenic San Gabriel mountains. Many physicians and other medical offices such as radiology, urgent care, sleep center and a pharmacy are located within the same building.
As one of the first rehabilitation clinics in the Chino Hills area, our experienced and specialized staff has been providing the Chino Hills community with quality rehab services for two decades. Located in the Pomona Valley Health Center at the corner of Grand and Chino Hills Pkwy, our Chino Hills clinic also offers Aquatic Therapy in our on-site Medicare approved indoor pool.
Offering both Physical and Occupational Therapy, our La Verne clinic also provides Aquatic Therapy in a large Medicare approved indoor pool. Another perk of this clinic is its spacious and airy exercise gym which gives patients a sense of relaxation as soon as they step through the door. But these aren’t the only advantages—ongoing communication with physicians and other medical services within the Pomona Valley Health Center offers patients piece of mind and convenience.
Dedicated exclusively to the rehabilitative needs of children, our Milestones clinics provide full service pediatric physical therapy, pediatric occupational therapy, and pediatric speech-language pathology. These unique clinics feature several specialized treatment areas, plus a large pediatric gym with specially designed rehabilitation equipment just for children. Our highly trained pediatric therapists are California Children's Services (CCS) paneled and licensed. Milestones is located at the Pomona Valley Health Centers in Chino Hills and in Claremont.
